Case Manager II: Disability Services & Care Coordination

Centers-For-Independence seeks a dedicated Case Manager II to coordinate care for individuals with intellectual/developmental disabilities, brain injuries, or medical support needs enrolled in Adult Day programming. The role emphasizes maintaining client records, stakeholder communication, and leading care plan conferences.

The ideal candidate will have a Bachelor’s degree in one of the related fields and relevant certification, with the ability to teach, adapt activities, and promote community
