Coordinator - START

Key Responsibilities Maintain an active caseload of up to 20–25 individuals referred for START Services; provide systemic consultation based on assessment and outreach findings, using ecomaps and best practices; consult with systems/teams as needed, escalating concerns to supervisor/Clinical or Medical Director. Conduct regular outreach visits to each individual's home, day, and/or vocational settings (in‑person and/or telehealth) to complete evaluations and assessments on an as‑needed or regular basis, based on level of support needed, activity level, and/or project scope. Assist with referrals for consultation and treatment, including screening for enrollment in START, completing intake assessments across the assigned region(s) consistent with applicable timelines and/or referrals to the clinical team, Medical or Clinical Director or Resource Center. Ensure coordination of support meetings and crisis plan development for individuals served through START, using the START Action Plan; provide information and results to and coordinate responses with Medical and/or Clinical Director, escalating concerns to the START clinical team as appropriate. Develop Comprehensive Service Evaluations, Cross‑Systems Crisis Prevention and Intervention Plans, intake/assessment, intervention and outcome plans for START Therapeutic Coaching Program, Resource Center admissions/discharges and other applicable documentation of services provided. Participate in recurring consultation, staff and other meetings with START leadership, clinical team and START Therapeutic Coaching program. Provide on‑call support on a regularly scheduled, rotational basis as part of the START team. Complete and maintain all applicable documentation, paperwork, or records on a timely basis, including resolution of concerns and reporting of incidents. Accurately enter all required data into the START Information Reporting System and other appropriate systems, as required by state, on a timely and contemporaneous basis. Conduct regular outreach with community partners to maintain linkages and relationships. Develop materials and present at Clinical Education Team Meetings as assigned by the director or designee. Participate in the Center for START Services National Network, including online resource center, conference calls, study groups and annual conference or other START events. Complete and maintain all necessary trainings to ensure compliance with YAI policies, NCSS and applicable state regulatory agency requirements (e.g., OPWDD, DDS) and obtain and maintain START Coordinator certification. Comply with all Federal, State, Local and other relevant regulatory agency requirements, including HIPAA. Perform all other duties as assigned. Minimum Qualifications Master’s degree in Mental Health, Psychology, Counseling, Social Work or related Human Service field. At least two (2) years of experience working with the I/DD population and/or behavioral health needs, or a satisfactory combination of education, experience and/or training as authorized by START/NCSS and/or appropriate State regulatory agency (e.g., OPWDD). All START Coordinators must have at least a bachelor’s degree in a human services discipline. Successful completion of START Coordinator Certification training within 12 months of appointment. Ability to provide on‑call coverage for crisis services on a regularly recurring basis, including weekdays, weekends, evenings and/or overnight on a 24/7 basis, as required. Ability to travel regularly across assigned region(s) to conduct in‑person assessments, evaluations and visits, and occasionally travel to other states as needed for NCSS meetings or events. Ability to work with complex systems and an interest in the population is essential. Exceptional interpersonal and problem‑solving skills. Strong communication and writing skills. Highly flexible personality type and willingness to take initiative. Preferred Qualifications Experience working with individuals with I/DD with co‑occurring diagnoses and complex behavioral needs. Experience working with people and their families and/or social service providers. Verbal and/or written fluency in Spanish (highly preferred); fluency in other languages may be preferred.
