Urgent Care Advanced Practice Provider

Role Description The Urgent Care Advanced Practice Provider is a full-time, on-site role based in Rochester, NY, focused on delivering timely, patient-centered care to older adults with urgent or acute needs. Day-to-day responsibilities include performing comprehensive assessments, diagnosing and treating common urgent conditions, ordering and interpreting diagnostic tests, and prescribing appropriate medications and therapies. The provider will coordinate closely with primary care clinicians, specialists, and caregivers to ensure continuity of care and appropriate follow-up after urgent visits. Additional duties include documenting encounters in accordance with regulatory and organizational standards, participating in care planning and case reviews, educating patients and caregivers on treatment plans and self-management, and supporting quality improvement initiatives within the urgent care setting. Qualifications Active Nurse Practitioner (NP) or Physician Assistant (PA) license in New York State and national certification as applicable. Demonstrated clinical skills in urgent care, primary care, or emergency care, including assessment, diagnosis, and treatment of acute and chronic conditions. Experience working with older adults or individuals with complex, chronic medical needs, preferably in home-based or value-based care models. Strong communication and interpersonal skills to collaborate effectively with patients, caregivers, and multidisciplinary care teams. Proficiency in electronic health record (EHR) documentation, clinical workflows, and adherence to regulatory and safety standards. Ability to exercise sound clinical judgment, prioritize urgent needs, and manage time effectively in a fast-paced care environment. Commitment to patient-centered, culturally sensitive care and continuous professional development.
