Personal Medical Assistant Scheduler Referral Coordinator Collaborative interdisciplinary care model Patient Population: Work with an appreciative senior patient population in a model designed to improve long-term health outcomes through preventative and personalized care. Physicians have the time and resources to build meaningful relationships with patients managing chronic conditions.
