Registered Nurse (RN) Case Manager | Homecare Rock Rapids

Location: Avera at Home Estherville Worker Type: Regular Shift: Primarily days with rotating weekends and holidays (United States of America) Pay Range: $30.50 - $45.75 (actual pay rate dependent upon experience) Position Highlights $10,000 sign on bonus Mileage reimbursement Student Loan Repayment Program: up to $10,000 with $167 monthly payment Responsibilities Functions as interventionalists in the home utilizing clinical expertise and problem solving to recommend physician interventions based on comprehensive patient assessments. Implements the nursing process utilizing assessment skills during Start of Care visits, post-hospital evaluations, patient re-certifications, discharges and supervisory visits. Partners with the physician and interdisciplinary team to develop patient-centered plan of care. Recommends efficient use of resources and applies clinical protocols during admission visits per patient diagnosis. Initiates and responds to requests for changes in plans of care and determines necessity of significant change requiring OASIS. Provides education to patients, families, and caregivers on disease process, self-care, and community resources. Demonstrates ability to influence care behavior and provide thorough instruction as ordered by the physician. Essential Qualifications Ability to work specified hours and perform essential job functions including visual acuity adequate to perform duties. Effective communication skills, perceive speech and other sounds clearly. Meet the knowledge, skills, and abilities required to perform essential job functions. Preferred Qualifications Registered Nurse (RN) – Board of Nursing credential at time of hire. 1-3 years of experience in home health or hospice.
