Case Manager (RN) - Inpatient - Relief A Status, 08-HR, Day Shift

$111.35 - $126.14 per hour

Overview Case Managers are licensed nursing professionals responsible for coordinating continuum of care and discharge planning activities for a caseload of assigned patients. They perform utilization review activities, ensuring the appropriate use of services, compliance with external review agencies, and the optimum use of resources. Case Managers act as consultants to the clinical team and other departments regarding patient assessment and care, and participate in program development and quality improvement initiatives. Location Stanford Health Care Responsibilities Coordination of Care – Manage each patient’s transition through the system and transfer accountability to the appropriate person upon entry into another clinical service or discharge. Discharge Planning – Coordinate and facilitate timely implementation of discharge plans for assigned patients with complex needs, collaborating with interdisciplinary team members and arranging follow‑up care. Utilization Review – Review all inpatients prospectively, concurrently, and retrospectively for appropriateness of admission, level of care, and length of stay. Monitor length of stay, collaborate with physicians to ensure resource utilization remains within covered benefits, and implement strategies to reduce length of stay and resource consumption. Education/Consultation – Provide educational resources and consultation regarding the discharge planning process, regulatory requirements, and case types to patients, families, and hospital personnel. Lead Work – Lead the work of administrative or clinical support staff assisting with case management for an assigned patient caseload. Other – Participate in department program planning, goal setting, systems development, and process improvement. Serve on committees and task forces, maintain documentation of findings, discharge arrangements, and actions taken, and collect, analyze, and report data for utilization, quality improvement, and compliance. Patient Assessment / Plan of Care – Collaborate with physicians, social workers, nurses, and other interdisciplinary team members to assess, plan, and coordinate patient care needs, ensuring consistent, timely, and appropriate care in a patient‑focused manner. Quality Improvement – Identify opportunities for improvement in clinical outcomes, resource utilization, and concurrent data collection; participate in clinical process improvement teams. Third‑Party Reimbursement – Collect, analyze reports, and review patient information with third‑party payers to assure reimbursement for patient services/procedures. Communicate with review organizations or payers to provide requested clinical and psychosocial information. Qualifications Education: Bachelor’s Degree from an accredited college or university required; Master’s Degree preferred. Experience: Three (3) years of progressively responsible and directly related work experience required. Knowledge, Skills and Abilities: Ability to collect and record data, evaluate data and statistics, develop and perform patient assessment and plan of care, monitor patient access to appropriate level of care, promote coordination and continuity in patient health care, provide age‑appropriate assessments, interpret data, deliver interventions, maintain knowledge of treatments and services, resolve conflicts, negotiate with others, understand federal and state compliance laws, work effectively with individuals at all levels, and maintain effective reporting systems. Licenses and Certifications: CA‑RN (Registered Nurse) required. Compensation Base Pay Scale: Generally starting at $111.35 - $126.14 per hour. The final rate will be set based on internal equity, experience, education, specialty and training.
