A specialized healthcare consulting organization dedicated to helping individuals and families successfully navigate the complexities of Medicaid eligibility, long-term care funding, and home care access. The organization operates within one of the most intricate areas of the healthcare systembridging the gap between patients in need of care and the financial and administrative pathways required to secure it.

This is a field-based Medicaid marketing and referral development role responsible for expanding and strengthening healthcare referral pipelines across Queens, Brooklyn, Manhattan, with additional reach into Nassau and Westchester counties. The role is centered on building meaningful, trust-based relationships within the post-acute and long-term care ecosystem, with a strong emphasis on home care agencies, skilled nursing facilities, and hospital discharge networks.

A critical differentiator in this role is the focus on hospital-based support groups rather than traditional cold outreach to hospital staff. Effective outreach will be concentrated within community-driven groups such as Alzheimer's, Parkinson's, dialysis, and chronic illness support networkswhere families actively facing care decisions are already engaged and receptive. This approach is designed to generate higher-quality referrals and improve conversion rates by meeting patients and families at the point of real need, rather than relying solely on administrative hospital channels.

The ideal candidate is already established within the home care or skilled nursing marketing space and brings a well-developed referral network, particularly within Medicaid-related services. Individuals with an existing book of business and strong relationships across nursing homes, admissions departments, Medicaid coordinators, and discharge planners will excel in this environment. This is a high-autonomy, field-driven role that rewards relationship depth, consistency of presence, and the ability to position Medicaid navigation services as a critical extension of the care continuum.

Responsibilities

• Develop and grow Medicaid referral pipelines across NYC boroughs, Nassau, and Westchester
• Leverage existing relationships within home care, skilled nursing, and long-term care networks
• Build and maintain strong partnerships with nursing home social workers, Medicaid coordinators, admissions teams, and discharge planners
• Focus outreach efforts on hospital-based support groups (e.g., Alzheimer's, Parkinson's, dialysis, chronic illness groups) to generate high-quality patient referrals
• Educate referral sources on Medicaid eligibility services and value-added consulting support
• Maintain consistent field presence across hospitals, SNFs, and community healthcare organizations
• Identify and cultivate new referral opportunities within assigned territory
• Track outreach activity, referral performance, and pipeline growth
• Collaborate with internal teams to ensure smooth client onboarding and case progression
• Represent the organization as a trusted Medicaid navigation partner within the healthcare community

Qualifications

• 310+ years of experience in healthcare marketing, preferably within home care or skilled nursing environments
• Strong existing network within NYC healthcare systems (hospitals, SNFs, home care agencies)
• Demonstrated experience generating referrals in Medicaid, long-term care, or related healthcare services
• Established book of business strongly preferred
• Deep understanding of healthcare referral processes, including discharge planning and Medicaid coordination workflows
• Strong relationship-building and communication skills with clinical and administrative stakeholders
• Ability to work independently in a field-based, territory-driven role
• Must be located in or near Queens, Brooklyn, Manhattan, Nassau, or Westchester
• Background in home care marketing is highly preferred over hospital-only outreach experience

Compensation

• Base salary: $60,000 $80,000
• Uncapped commission structure based on referral volume and performance
• Strong earning upside for well-networked candidates with established referral pipelines
