Position Summary

The Care Advocate II provides Enhanced Care Management (ECM) and Community Supports (CS) services to Medi-Cal members with complex medical, behavioral health, housing, and social needs.

This experienced-level position manages an independent caseload while coordinating care across healthcare providers, behavioral health professionals, housing agencies, and community organizations. Care Advocate II staff serve as trusted advocates for members while helping them navigate systems of care and overcome barriers related to the Social Determinants of Health (SDOH).

In addition to direct member services, Care Advocate II staff support quality improvement initiatives, community outreach, and mentorship of newer advocates as part of EPIC's collaborative care team.

What You'll Do

As a Care Advocate II, you will:

• Manage a full caseload of members with complex health and social service needs.
• Conduct comprehensive assessments and develop individualized care plans.
• Coordinate services with healthcare providers, behavioral health professionals, housing partners, and community organizations.
• Deliver Enhanced Care Management (ECM) and Community Supports, including Housing Transition Navigation and Housing Tenancy & Sustaining Services.
• Connect members with healthcare, housing, transportation, food resources, benefits, and other community supports.
• Advocate for members while promoting self-advocacy, independence, and long-term stability.
• Maintain timely, accurate, and compliant documentation using agency systems.
• Meet productivity, quality, and documentation standards established by EPIC and CalAIM.
• Support outreach events, enrollment activities, and other organizational initiatives.
• Provide mentorship and peer support to newer advocates and contribute to a positive team culture.

Minimum Qualifications

• High school diploma or equivalent.
• One to two years of experience in case management, care coordination, social services, healthcare, behavioral health, housing services, or a related field.
• Experience working with vulnerable or underserved populations.
• Strong organizational, communication, and documentation skills.
• Ability to work both independently and collaboratively.
• Valid California driver's license, reliable transportation, and proof of insurance for field-based responsibilities.

Preferred Qualifications

• Bachelor's or Master's degree in Social Work, Psychology, Sociology, Human Services, Public Health, or a related field.
• Experience with Medi-Cal, CalAIM, Enhanced Care Management (ECM), Community Supports, healthcare, housing, or nonprofit programs.
• Knowledge of Monterey County community resources and social service systems.
• Bilingual (English/Spanish) strongly preferred.

Work Environment

This position is primarily office-based with regular fieldwork throughout Monterey County. Care Advocate II staff routinely meet members in homes, healthcare settings, community organizations, shelters, and other community locations.

Travel throughout the county is required as part of providing person-centered services.
