Crisis Behavioral Health Clinician: Care & Transitions

Community Bridges, Inc. is seeking a Behavioral Health Clinician (BHC) to join their interdisciplinary treatment team. The BHC will coordinate care with Patient Care Technicians, Registered Nurses, and Behavioral Health Medical Providers to ensure safe and effective service delivery. This role includes conducting behavioral health assessments, creating individualized service plans, and collaborating with families during transitions. A Bachelor’s degree in behavioral health is required, and experience in mental health settings is preferred.
