Physical Therapist

Job Title: Home Health PT - Physical Therapist Location: Cleveland, TN Status: Full-Time Position Overview The Physical Therapist is responsible for the assessment and evaluation of patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Based on the assessment, the Physical Therapist develops and implements a treatment plan, performs interventions to improve or maintain patient well‑being, and evaluates progress. Key Responsibilities Evaluate the patient's functional status and develop individualized therapy needs. Assist physicians in developing the therapy plan of care. Observe and report patient response to treatment and any changes in condition to supervising nurse or physician. Treat patients to relieve pain, restore function, and maintain maximum functioning using standard practice. Inform patients, families, and care team of the plan of care, functional limitations, and progress toward goals. Qualifications Current PT license in the state where services are rendered. Current CPR certification. Valid driver's license, vehicle insurance, and reliable vehicle or public transportation for daily commutes. Ability to reliably commute to multiple patient residences within the assigned geographic area.
