About the Position 

The Medical Appointment Coordinator/Medical Clerk is responsible for coordinating and scheduling medical, dental, psychiatric, therapy, and other healthcare appointments for individuals receiving services. The position helps ensure appointments are scheduled in a timely manner, communicated to the appropriate departments, and accurately tracked to promote continuity of care.

Essential Job Responsibilities

• Schedule routine, follow-up, annual, and specialty medical appointments for assigned clients.
• Coordinate appointments with physicians, dentists, specialists, hospitals, clinics, laboratories, pharmacies, and other healthcare providers.
• Maintain an organized medical appointment calendar and monitor upcoming appointments.
• Schedule appointments far enough in advance whenever possible to allow for appropriate staffing and transportation arrangements.
• Communicate appointment dates, times, locations, and relevant instructions to Residential, CDS, QIDPs, Health & Wellness staff, and other appropriate team members .
• Notify appropriate staff promptly when appointments are scheduled, canceled, or rescheduled.
• Assist with coordinating transportation needs related to appointments.
• Track appointments that require follow-up and ensure recommended follow-up appointments are scheduled.
• Contact medical offices when clarification or additional scheduling information is needed.
• Assist with obtaining appointment paperwork, referrals, orders, or other required documentation as appropriate.
• Maintain accurate records of scheduled and completed appointments.
• Update appointment information in the organization's designated electronic or paper tracking systems.
• Identify scheduling conflicts and communicate them to the appropriate supervisor or department.
• Follow up regarding missed appointments and assist with rescheduling.
• Maintain confidentiality of client health information in accordance with HIPAA and organizational policies .
• Communicate professionally with guardians, healthcare providers, staff, and interdisciplinary team members as authorized and appropriate.
• Immediately report urgent medical information or concerns received during scheduling to the appropriate nurse or supervisor rather than attempting to interpret or address the concern independently.
• Complete other administrative duties related to medical appointment coordination as assigned.

Location & Schedule 

Location: Des Plaines, IL 

Schedule: Monday – Friday, 9AM - 5PM

Preferred Qualifications

• College degree
• Microsoft Office experience (1+ year)
• Customer service experience (1+ year)

Requirements 

You must meet the following requirements: 

Pass: 

• A fingerprint criminal background check 

• A child abuse and neglect check 

• A drug screen. 

Provide:

• An original document (no photocopies) showing high school completion or high school equivalency. Examples include: High school diploma, GED certificate, college transcript 

• Residency for the past 2 years 

• Current & original documents that verify your identity & ability to legally work in the United States 

• Two verifiable employment references within the last 3 years
