The ideal candidate has strong clinical judgment, outstanding communication skills, and a deep commitment to patient-centered care. You’ll be part of a collaborative, supportive team that treats each other like family while delivering the highest standard of home-based therapy. What You’ll Do Conduct Speech Therapy Evaluations and assessments to establish individualized plans of care Evaluate and treat patients with speech, language, cognitive-communication, voice, and swallowing disorders Develop and implement personalized treatment plans to improve communication, cognition, and safe swallowing Provide therapy for dysphagia, including patient and caregiver education on safe swallowing strategies and diet recommendations Educate patients and caregivers on communication strategies, home exercise programs, and compensatory techniques Collaborate closely with physicians, nurses, therapists, and other interdisciplinary care-team members to coordinate care Monitor patient progress, update care plans, and maintain thorough, timely, and compliant documentation Ensure compliance with all Medicare, CMS, and home health standards What We’re Looking For Current Florida Speech-Language Pathologist license (required) Certificate of Clinical Competence (CCC-SLP) preferred Previous home health experience is strongly preferred but not required Excellent communication, time management, and documentation skills A compassionate, dependable, and professional attitude A team-oriented mindset and a genuine passion for improving patients’ quality of life What Sets Us Apart A true culture of teamwork and family: we support one another and celebrate each success together A patient-first approach that respects patient preferences and promotes dignity Work-life balance with flexible scheduling, so you can care for your patients and your life outside of work Reputation as the best in home healthcare in our area, built on trust, quality of care, and long-standing relationships with patients and families Are you ready to join an Awesome Team?
