What You’ll Do

• Coordinate and process incoming home health referrals
• Gather and verify patient, insurance, and referral information
• Enter accurate referral information into the electronic medical record (EMR)
• Communicate with hospitals, physicians, facilities, insurance case managers, patients, and team members
• Help coordinate appropriate start-of-care dates
• Follow up on incomplete referrals and help keep the intake process moving
• Manage incoming fax and website communications

• Medical terminology coursework or equivalent education/experience
• Medical office experience required
• Nursing background (CNA, MA, Med Tech) welcomed, but not required
