For more information, please visit Program Overview In partnership with Arizona's Department of Children's Services (DCS), Youth Villages provides support to young people (ages 17 to 23) wanting to maintain eligibility for extended foster care services through the Extended Foster Care (EFC) Program. The goal of EFC is to ensure better outcomes for young people as they transition into adulthood. The program offers individualized, clinically focused case management and counseling support across key areas: interpersonal connectedness/social support, housing, mental and physical health, career and employment, life skills, and education. At its core, EFC equips young people with the self-sufficiency and self-advocacy skills needed for long-term success after foster care. Position Overview The EFC Success Coach engages youth in a coach-like way and provides strengths-based case management and interventions as needed for youth and young adults preparing for and participating in the Extended Foster Care Program. Essential Duties and Responsibilities Carries a caseload of up to 20 young adults Maintains weekly communication and meets with each youth/young adult in-person 1 to 3 times per month (depending on need) Builds a supportive relationship with youth/young adult, works to develop goals and a plan for achieving those goals, and monitors/adjusts the plan based on progress and need areas Utilizes interventions to address the following domains: housing, education, natural supports and healthy relationships, employment, health, transportation, and money management Acts as a liaison with community service providers and other key stakeholders Participates and/or facilitates team-based planning meetings as needed Empowers young adults to build a strong support system by utilizing their natural environment and community resources Provides transportation assistance as needed Attends 3 weekly meetings (individual, team, and clinical consultation) for professional development to help enhance clinical skills Conducts on-going assessment of young adult to determine their needs from a strength-focused, solution-based perspective Engages and aligns with the young adult and their support system to elicit full participation in treatment Provides on-call crisis support to the young adult (schedules vary by location) Completes accurate and timely documentation in an electronic medical record system (EMR) Performs other duties as assigned Additional Information Schedule is flexible and non-traditional as it is based around the availability of youth and families served. Applicants must possess a current, valid driver’s license, an automobile for work purposes, and proof of auto insurance. Community-based staff will be reimbursed for applicable mileage. Salary: $58,000 - $66,000 based on education and license. Qualifications Master’s degree in a social services discipline (preferred) Bachelor's degree in a social services discipline (required) Degrees that can be considered include: social work, drug and alcohol education or counseling, psychology, criminal justice, guidance counseling, or marriage and family therapy (others subject to review) Experience serving transition age youth (preferred) Experience in systems of care including foster care, juvenile justice, runaway, and homeless youth programs (preferred) Clinical experience (preferred) Strong organizational skills and attention to detail Excellent written, verbal, and oral skills Ability to manage multiple priorities simultaneously Basic computer knowledge Ability to maintain a flexible schedule Youth Villages Benefits Medical, Dental, Prescription Drug Coverage and Vision 401(k) Time off: 2 week paid vacation (full-time) / 1 week paid vacation (part-time) 12 paid sick days per year 11 paid holidays Paid Parental Leave Mileage & Cell Phone Reimbursement (when applicable) Tuition reimbursement and licensure supervision Growth & development through continuous training Clinical and administrative advancement opportunities *Benefits are excluded for variable status employees.
