Intensive Housing Support Specialist

The schedule for this shift would be as follows:Saturday-Monday, 3:00PM-1:00AM (0.75 FTE)Our Intensive Housing Specialist will work as part of a team to provide comprehensive community supports for young adults in transition to community based supported housing programs with emphasis on recovery-oriented services. Primary duties involve assisting clients with making sustainable and reliable community based support networks, learn daily living skills to promote long term permanent housing, and empower clients to become active members of their residential environment.At Community House, we value a strong work-life balance and aim to provide the best support for our workers. As a CHMHA employee, you will have access to a range of unique benefits, such as company-paid medical/dental/vision/disability insurance, a 403B retirement plan with employer match, continuing education credit, a personal unlimited ORCA card, and an employee assistance program with financial, mental, and legal assistance.RESPONSIBILITIESProvide assistance in accordance to individual treatment plans such as hygiene needs, meal preparation, daily skills groups, and community outingsAssess living spaces for cleaning purposes on a weekly basisCoordinate client needs such as appointments, engagement with community support networks, vocational services, or other applicable needsProvide immediate crisis intervention and resolution as neededReview daily log and documents after each shift and communicate other staff members of any emergency situationsVisually supervise medication distribution as necessaryLead assigned life group skillsMaintain client records in keeping with agency, county, state, and federal guidelinesProvide input to primary psychiatric provider and/or case manager on client progress and treatment planFacilitate assigned group activity sessions and specialized client events such as field trips, holiday parties, etcAssist in admission and discharge planning to community based supported housing programs when directedCoordinate client orientation and monitor/inform property manager of room maintenance as neededAttend scheduled staff meetings and community meetings when assignedCommit to minimum 10 hours of annual continuing education/trainingMaintain client and staff confidentiality at all timesBASIC QUALIFICATIONSHigh School Diploma/GED with minimum 2 years experienceCurrent First Aid and CPR certificationCurrent Food Handler permitHave legible handwritingAbility to do simple mathematicsAbility to read and comprehend agency/client chartsValid driver’s license and clean driving record for use of agency vehiclesExperience with target population or ability to acquire specialized training to work with target population.Ability to work independently and in a team.Be free of substance useAbility to perform duties outlined in description within reasonable accommodationPREFERRED QUALIFICATIONSBachelor’s degree in social work or other related fieldWorking knowledge and adherence to all agency policies and procedures #J-18808-Ljbffr Mental Health
