- Perform clinical social work duties including assessments, referrals, and patient/family counseling.
- Collaborate with physicians, nursing staff, and other care professionals to deliver holistic patient care.
- Provide leadership and mentoring to fellow social workers within the department.
- Contribute to care planning and discharge coordination for complex cases.
- Master's Degree in Social Work (MSW) from an accredited institution.
- Active California Clinical Social Worker License (LCSW) upon hire.
- Minimum 5 years of clinical social work experience , including 4 years in a healthcare setting .
- 4 years of supervisory or management experience in a social services or healthcare setting.
