Hospice RN Case Manager — Meaningful, Local Patient Care

Suite Living Senior Care

Suite Living Hospice Care is seeking a dedicated RN Case Manager to join our hospice team. You will manage a caseload of 10–15 patients, coordinating care with an interdisciplinary team to ensure comfort, dignity, and quality of life in the northwest metro. This full-time role offers a Monday–Friday schedule with limited on-call, a defined territory in Eden Prairie, Plymouth, and Crystal, and competitive pay along with Blue Cross Blue Shield medical plans, PTO, 401(k), and mileage reimbursement. #J-18808-Ljbffr Suite Living Senior Care
