HOME HEALTH AIDE JOB DESCRIPTION

Reports to:  Home Health Supervisor                                                  Department: Advocate Homehealth

Classification:     Full-time/hourly non-exempt                                     Division:  Home Health

JOB SUMMARY:

Home Health Workers will provide services to authorized Members as dictated by their approved Care Plan.  These services include but are not limited to: homemaking, personal care, respite, chore, companionship.

DUTIES AND RESPONSIBILITIES:

Maintain professional, on-time delivery of services as defined below:

HOMEMAKING: Homemaking services are home management services performed by trained homemaker which includes housekeeping, laundry, cleaning refrigerators (spills), clothing repairs, meal planning and preparation, shopping assistance, and routine household activities.

PERSONAL CARE:  Personal Care is assistance with eating, dressing, personal hygiene and other activities of daily living.  Home Health Service Workers may assist with meal preparation, housekeeping chores such as bed making, dusting and vacuuming incidental to the care furnished or essential to the health and welfare of the individual.  Personal Care also includes shopping assistance to purchase food, clothing and other items needed required for the client’s health and well-being.

RESPITE:  Home Health Workers can provide this service in the client’s home or any safe environment suitable to the needs of the client.  Respite may include personal care, homemaker, or companionship activities as dictated by the approved Care Plan.

CHORE:  Home Health Workers will provide routine house chore as seasonal cleaning.  These activities include cleaning closets, cleaning interior windows cleaning out refrigerators and stoves, cleaning off window seals and cleaning ceiling fans.  All services are contingent on proper equipment being provided to ensure worker safety.  Chore is provided as dictated by the Care Plan.  If client is in need of more Chore services than Care Plan states, the Home Health Worker should contact supervisor for further instruction.

COMPANIONSHIP:  The Home Health Worker visits Members who are socially and/or geographically isolated.  The visit is designed to relieve loneliness and provide continuing social contact with the community by casual conversation, assistance with reading, writing letters and entertaining games.

Immediately inform Home Health Supervisor of any change in Members living condition, health, vacations, hospital stays or any other status change that effects service.

Turn in all time sheets and mileage to Home Health Supervisor no later than Monday at 10am every week.

Contributes to the team effort by accomplishing related results as needed.

KNOWLEDGE, SKILLS AND ABILITIES:

Administrative writing skills.

Microsoft Office skills.

Strong Verbal communication skills.

Professionalism

SUPERVISION:

Performs duties under the supervision of the Home Health Supervisor.  This position does not supervise other personnel.

WORK EXPERIENCE/REQUIREMENTS:

High School diploma or GED (Job experience can be substituted and applicants have one year to complete their GED).

Certified as a Nursing Assistant (CNA).

Experience working with the elderly in a customer focused and fast paced professional environment.

Must be able to pass Level 2 background screening required by DEOA.

Must adhere to NCCOA Drug Free Workplace Policy.

EDUCATION:

AA or college course work and appropriate work experience.

PHYSICAL WORK REQUIREMENTS:

Able to sit and work at a computer keyboard for extended periods of time.

Able to stoop, kneel, bend ante the waist and reach on a daily basis.

Able to perform general office administrative activities:  copying, filing, delivering and using the telephone.

Able to lift 50 pounds.
