Licensed Vocational Nurse

Assess, plan, and implement the nursing care of residents within the skilled nursing setting with supervision from a Registered Nurse and Director of Nursing. Responsible for administering medications following the eight medication rights to reduce the potential for medication errors. Ensures continuity of care for residents between shifts by providing direct care and supervising the care given by supportive staff members. Participates in resident and family teaching. Maintains the role of resident advocate with a focus on the facility's mission. Assesses residents' conditions and nursing needs and prescribes appropriate nursing actions. Demonstrates the ability to adequately assess and reassess pain and utilize appropriate pain management techniques. Educates residents and families regarding pain management. Documents changes in residents' conditions and nursing care in progress notes and care plans. Performs waived testing per policies and procedures. Interprets results of waived tests and takes appropriate action based on results. Administers medications while adhering to the eight medication rights. Meets current documentation standards and policies. Notifies physicians of changes in residents' conditions and follows through until appropriate action is taken. Notes and conducts physician orders in a timely manner. Gives thorough reports to oncoming shifts through walking rounds. Acts on information received during shift reports regarding resident concerns. Implements the plan of care for residents based on assessments and goals set by the interdisciplinary care team. Treats residents and families with respect and dignity while addressing their psychosocial, cultural, ethnic, and spiritual needs. Supervises other members of the healthcare team as needed to achieve the established plan of care. Demonstrates competence in technical and manual skills according to the scope of practice for LVNs. Functions as a resident advocate and protects residents' rights. Demonstrates creativity in problem-solving and innovative thinking. Interacts with residents, families, co-workers, and management in a friendly, knowledgeable manner. Involves residents and families in planning for care, treatment, teaching needs, and discharge plans. Serves as a leader for supportive staff and delegates responsibilities to enhance the excellence of the nursing unit. Participates in the orientation of new personnel and assists in performance evaluations when needed. Follows proper infection control procedures and supervises supportive staff in infection control protocols. Maintains composure in emergencies and stressful situations, providing a sense of confidence to those involved. Consistently follows facility policies and procedures. Exhibits genuine interest and appreciation for adults in later years of life. Demonstrates motivation and the ability to organize workloads to ensure all functions are completed appropriately.
