Administer medications following the eight medication rights to reduce the potential for errors. Ensure continuity of care between shifts by providing direct patient care and supervising CNAs and supportive staff. Document changes in patient condition and nursing care on progress notes and care plans; maintain accurate records. Perform waived (point-of-care) testing per policy; interpret results and take appropriate action. Notify physicians of changes in patient condition and follow through until appropriate action is taken; participate in thorough shift handoffs and care coordination.
