Home Health Physical Therapist Assistant

We are seeking a Physical Therapist Assistant to deliver patient treatment, exercise instruction, and education to enhance patient well-being within a home care setting. Key Responsibilities Include Following state-specific laws and supervising physical therapists to provide safe, qualified care. Instructing patients in active/passive exercises, gait, ADLs, transfers, safety, and prosthetic training. Monitoring, documenting, and communicating patient responses and condition changes to the healthcare team. Educating patients, families, and caregivers on therapy practices within scope of practice. Qualifications Required Current licensure/certification as a Physical Therapist Assistant CPR certification Valid driver’s license, reliable transportation or access to public transit Ability to work effectively with diverse patient populations and home environments Preferred skills: Physical strength for patient mobility tasks, including lifting and transfers. The role offers competitive hourly pay ($28.00 - $42.00).
