Our mission is to provide extraordinary care by empowered people through exceptional outcomes.Recently awarded among Arizona’s Most Admired Companies in 2022 & 2023Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Drive Recovery Health Center.NEW Competitive CompensationOn-call EmployedLocation: 23rd Ave & DunlapBilingual in Spanish is a plus (Additional Language Differential Pay Available)No experience required with a bachelors degree in behavioral health - we will train youNew grads welcomeDuties may include:Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goalsCompletes assessments, crisis plans and individualized service plansWorks collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implementedConducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluationsPursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenureEducation/Experience:GED/High school diploma with a minimum of 1.5 years of behavioral health experience, or,An associates degree in the field of medical or behavioral, orA bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, orA bachelor's degree in the field of behavioral, orA master's degree or higher in the field of behavioral healthComputer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferredMinimum typing speed of 25 wpmMust have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policyMust have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employmentInterpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community membersGood communication skills, evidenced by a non-judgmental approach, effective listening and positive interactionsBasic understanding of the DSM-V preferredCertified Case Manager strongly preferredMust pass background check, TB test and other pre-employment screeningPhysical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job.
