About the Position

Job Title: Case Manager Profession: Registered Nurse Specialty: Care Management Duration: 12 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 3 years License: GA or Compact license required Certifications: BLS Must-Have: Ability to assess, evaluate, and drive discharge plans. Clinical experience in care coordination. Description: Ability to assess, evaluate, and drive discharge plans. Coordinate services to the patient's appropriate level of care. Ensure that the treatment plan is initiated, evaluated, and re-evaluated daily. Follow the progress and lack thereof daily. Monitor length of stay and identify barriers to discharge. Follow regulatory guidelines regarding patient choice and notifications. Schedule: Monday to Friday. Weekends: None. Call: None. Floating: Within facility or scope of practice as needed. Travel experience required. Full compliance required to start on time; if not compliant in time, there will be a delay. All time off requests must be finalized at the time of submission. Key holidays include Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas, and New Year's. Perform clinical screens to determine clinical fit. Interdisciplinary support provided. Daily census between 20-25 patients. Community hospital experience preferred. Experience in long-term acute care or skilled nursing is a plus. Must demonstrate knowledge of regulatory and compliance standards. Maintain confidentiality in accordance with privacy guidelines. Ability to work independently and as part of a team. Strong communication and organizational skills are essential.
