About the Position

Job Title: Registered Nurse Case Manager Profession: Nursing Specialty: Case Management Duration: 12 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: Minimum 3 years License: GA or Compact license required Certifications: BLS Must-Have: - Ability to assess, evaluate, and drive discharge plan - Coordination of services to the patient's appropriate level of care - Ensuring that treatment plan is initiated, evaluated, and re-evaluated daily - Monitoring length of stay and identifying barriers to discharge - Following applicable regulatory guidelines regarding patient choice Description: The role involves assessing and evaluating discharge plans for patients. You will coordinate services to ensure patients receive appropriate care levels. It is essential to initiate, evaluate, and re-evaluate treatment plans on a daily basis. Daily monitoring of patient progress and barriers to discharge is crucial. You will adhere to relevant regulatory guidelines concerning patient choices and notices. There is a required schedule of Monday through Friday, with no weekend or call obligations. Flexible floating within the facility, as per practice scope, may be required. The unit does not accept first-time travelers and requires clinical screening for fit. If major holidays fall during your assignment, you may be expected to work on those days. Compliance to onboarding requirements is necessary to start on time.
