This role is onsite, in a supportive and collaborative home care facility in Buffalo, New York. Key Responsibilities

• Conduct comprehensive psychiatric assessments and evaluations to accurately diagnose various mental health conditions, ensuring a thorough understanding of each patient's needs.
• Manage medication prescriptive practices by providing ongoing medication management and closely monitoring the effectiveness of treatments, adjusting as necessary to achieve the best patient outcomes.
• Collaborate effectively with interdisciplinary team members, including primary care providers, to deliver integrated care that addresses both psychological and physical health needs.
• Provide timely crisis intervention and support to patients experiencing emotional distress or acute psychiatric episodes, ensuring safe and appropriate care pathways.
• Engage in community outreach and educational initiatives designed to elevate mental health awareness, aiming to destigmatize mental health challenges and promote access to care.
• Deliver therapeutic support to patients through counseling and psychoeducation, facilitating constructive coping strategies and skills development.
• Maintain meticulous and confidential patient records in compliance with HIPAA regulations, ensuring that all documentation is accurate and up-to-date.

Qualifications

Compensation and Benefits
