Job Summary: Provide specialized physical therapy services to patients in a skilled nursing facility, focusing on restoring and improving mobility, strength, and functional ability. Collaborate with healthcare teams to develop and implement individualized treatment plans.

Responsibilities:

• 
  Assess patients physical conditions and develop personalized treatment strategies.

  Implement therapeutic interventions to improve patients mobility and functionality.
  
  
• 
  Monitor and evaluate patient progress, adjusting therapy plans as needed.
  
  
• 
  Educate patients and their families on exercises, mobility techniques, and recovery goals.

  Document patient outcomes and maintain accurate treatment records.
  
  
• 
  Comply with all regulatory and safety standards while delivering quality care.
  
  

Qualifications:

• 
  Valid state license or certification as a Physical Therapist.
  
  
• 
  Proven experience working in a skilled nursing or similar facility preferred.

  Strong knowledge of orthopedic, neurological, and geriatric therapies.

  Excellent communication and interpersonal skills.
