IHSS Clinical Program ManagerThe IHSS Clinical Program Manager is a qualified registered nurse responsible for overseeing and coordinating clinical activities for a designated group of patients and staff specific to the Colorado IHSS program. In this role, you will report to the Director of Nursing (DON).Duties/Responsibilities:Supervises and evaluates a group of qualified Attendants to deliver client-centered careProvide supervisory support for clients in collaboration and as agreed upon by the client, Authorized Representative, and Licensed Medical ProfessionalAdministers skill validation for Attendants and documents their competency in IHSS and basic client safety proceduresEnsures assessment and correction of skills validation factors for assigned staff membersCounsels Attendants and staff on complex cases and potentially dangerous situationsCollaborates with Clients, Authorized Representatives, and Attendants in the event of medical issuesParticipate in activities related to the provision of professional services for assigned clientsMaintains compliance with agency policies and procedures, laws, regulations, and professional standards of practiceCoordinates client care services for assigned clients with the Authorized Representative, the Case Management Agency, the Licensed Medical Professional, the client, and other Home Health Agencies to ensure services continue without interruptionReview the care plan, Physician Attestation, and any documentation provided by the Case Management Agency during the initial enrollment, with any change in condition and upon request of the client/Authorized Representative or the Case Management AgencyProvides services in accordance with the plan of careAssists DON with development/implementation, and keeps records of criteria for admission to service, case assignments, and case managementReports to the DON any changes that might affect the agency's operationsPerforms coordination as assigned during emergencies and understands the Emergency PlanInvestigates, reports, and documents complaints and critical incidents according to regulatory timelinesMaintains confidentiality of agency, employee, and patient mattersAssists in orientating new staff prior to client interactionsVerifies Attendant compliance with care plan tasksSubmits documentation within required timeframesSupports after-hours on-call duties related to client safetyPerforms additional duties as assigned by the DONRequired Skills/Abilities/Knowledge:Evidence of adequate health status to perform dutiesAbility to learn, absorb and apply professional trainingAbility to organize work procedures, assume responsibility and effectively supervise othersDemonstrates excellent problem-solving techniques and abilitiesDemonstrates effective knowledge of the federal and state regulations governing home health agencies and IHSS providers with the ability to ensure complianceAble to see and hear adequately to respond to auditory and visual requests which relate to the coordination of clinical activitiesAble to speak in a clear, concise voice in order to communicate adequately with staff, physicians, and patientsAble to effectively deal with changeExcellent computer skills requiredEducation/Experience/Licenses/Certifications:Graduate of an approved school of nursing licensed in the state of practice; that has at least one to three (1-3) years of experience as a registered nurse in a state licensed home health agency, or other health care facilityOne year of home health and IHSS program experience (preferred)Undergraduate degree (preferred)Physical Requirements:You are not required to disclose information about physical or mental limitations that you believe will not interfere with your ability to do the job.
