Provider Type: Physician Coverage Type: Scheduled Clinical Hours Only EMR System: Epic Practice Setting: Outpatient Patients Per Shift: 10-15 Does your Facility require physicians to be board certified?: Yes Fellowship: NA Minimum Board Certification: Eligible Estimated Credentialing Timeframe (Days): 90 What is the license requirement to bid?: Licensed in CA Provider will work 2 days per week in the outpatient surgery center next door and 3 days per week in the clinic setting. All epidurals, facet injections, ablations, and other procedures are performed at the surgery center. We do prefer candidates that can give full time availability. Must have active CA license and must be Board Certified or BE within 5 years of residency. Active FLURO is required. We will look at candidates that have the temp fluro in hand
