Nurse Case Manager

Job Summary Job Description Summary The Nurse Case Manager (NCM) II reports to the Manager of Case Management and Care Transitions. Under guidance of the Nurse Case Manager Team Leader, the Nurse Case Manager I functions as a member of the clinical team. This role coordinates a plan of care to achieve clinical outcomes and promotes a safe and timely discharge. This position manages the plan of care throughout the care continuum to ensure the efficient movement of the patient through the hospital system. The Nurse Case Manager II serves as a clinical expert related to discharge planning, resource utilization, and case management. Qualifications Minimum Training and Education Bachelor's degree in Nursing from an accredited school of nursing and three years work experience as a Registered Nurse required. Prior leadership experience preferred. One year of Case Management experience preferred. Case Management certification preferred. Required Licensure, Certifications, Registrations Licensure as a registered nurse by the state of South Carolina or compact state required. Certification in Case Management preferred. Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required. Physical Requirements Ability to perform job functions while standing, sitting, walking, and climbing stairs. Ability to work indoors and occasionally outdoors in temperature extremes. Ability to work from elevated and confined spaces. Ability to perform procedures from kneeling positions. Ability to bend, twist, and squat at the waist. Ability to perform pinching operations and gross motor activities with fingers and hands. Ability to perform repetitive motions with hands, wrists, elbows, and shoulders. Ability to fully use both legs and lower extremities for balance and coordination. Ability to reach overhead and in all directions. Ability to lift and carry 50 lbs. unassisted and lift/lower objects 50 lbs. from the floor. Ability to lift from 36 inches to overhead 25 lbs. Ability to exert up to 50 lbs. of force. Ability to recognize objects close at hand and at a distance, match colors, and determine distance/relationships; good peripheral vision. Ability to maintain hearing acuity with correction. Ability to deal effectively with stressful situations. Availability to work rotating shifts, overtime, and a latex safe environment.
