Nurse Practitioner Opportunity - 25 visits per week - $140k+ salary! Job Title: Primary Care Nurse Practitioner – Advanced Practice In-Home and Care Center Provider Location: Greater Phoenix Metro Area, AZ (Targeting Tempe, South Scottsdale, Mesa, Chandler, Gilbert, and Central Phoenix) Schedule: Flexible, during the hours of Monday – Friday, 8:00 AM – 5:00 PM | Cluster Scheduling & Flexible Dedicated Admin Time Work Model: Field-Based / In-Home Care (Up to a 50-mile travel radius from home base) Position Overview We are seeking an autonomous Nurse Practitioner (Family Medicine or Adult-Gerontology) to serve as a Primary Care Provider (PCP) of record for members across Maricopa County. You will deliver comprehensive facility-based (SNF, Assisted Living) and in-home primary care, manage chronic conditions and facilitate long-term health outcomes. Key Responsibilities Direct Patient Care: Perform Community Bases Assisted Living or Skilled Nursing Facilities and in-home visits (minimum 25 visits/week), comprehensive assessments, physical exams, and medication reconciliations, with dedicated medical Assistant/RN support. Longitudinal Primary Care: Serve as the PCP of record, managing complex, chronic conditions and implementing individualized treatment plans. Collaborative Support: Partner with interdisciplinary care teams and leverage dedicated RN/MA clinical support for administrative and inbox management. Schedule & Territory Management: Complete cluster-scheduled home visits within a 50-mile radius of your residence and manage flexible administrative hours. Call Coverage: Participate in a rotational call schedule (Monday–Friday, transitioning to 7-day coverage blocks roughly once every 5 weeks when fully staffed). On-call duties receive additional compensation above base salary. Qualifications & Experience Licensure: Active, unrestricted Nurse Practitioner (FNP or AGNP) or Physician Assistant license in Arizona. Autonomous provider status required (or achievable within 6 months of eligibility). Certifications: DEA registration and current BLS certification. Experience: 3–5 years of Primary Care or Adult Outpatient experience strongly preferred (minimum 2+ years required). Strong background in Family Practice or Adult Medicine (Psychiatry and Women’s Health backgrounds are not applicable for this focus). Candidates with 1–2 years of primary care/outpatient experience and a strong Registered Nurse background will be considered. Driving & Travel: Valid driver’s license, reliable transportation, clean driving record, and willingness to travel within Maricopa County. Technical Skills: Proficiency with EMR documentation and risk adjustment concepts (CMS / ACA). Working Conditions & Remote Setup Requirements Home office setup with high-speed internet (minimum 20 Mbps download / 5 Mbps upload) for administrative tasks. Capability to transport medical equipment (ability to lift up to 20 lbs) and navigate local travel in varying weather conditions. Market-competitive salary commensurate with experience and qualifications ($140k - $145k, DOE), plus annual bonuses.
