Job Details Location: Seattle, WA 98144 Salary Range: $30.00 - $36.47 per hour Job Type: Full‑time, Monday‑Friday, 9:00AM – 4:00PM (35 hours per week) Organization: Community House Mental Health Agency Position Overview At Community House Mental Health Agency, we strive to provide a supportive family atmosphere in Seattle and King County for people with severe and persistent mental illness. Benefits Company‑paid medical, dental, vision, and disability insurance 403B retirement plan with employer match Continuing education credit Personal unlimited Orca Card Employee assistance program (financial, mental, and legal assistance) Responsibilities Serve as Primary Case Manager for assigned caseload, including advocating for and coordinating client needs and services; ensuring Service Plans, Crisis Plans, and other documentation are completed on time and kept up‑to‑date, and providing consistent contacts, skills training, and care coordination to work toward the client, program, and contract goals. Coordinate with relevant internal and external departments to coordinate client referrals, ensure timely intake into program, and develop individualized plans. Collaborate with the care team to ensure all clients access needed treatment, support, case management, and appointments, including transportation to external providers. Outreach to clients in their homes or community to assess living situation and stability, and provide treatment interventions when necessary. Ensure all interactions represent the five principles of trauma‑informed care—safety, choice, trustworthiness, collaboration, and empowerment. Coordinate access to medications for all clients through trained staff, ensuring the seven rights of medication use are followed: right person, right medication, right time, right dose, right route, right reason (including refusal), and right documentation according to agency‑provided medication training. Complete daily documentation in the electronic medical record on client interactions for billable and non‑billable activity, medications dispensed where applicable and assigned, and other required documentation—ensuring interactions and documentation are completed for all assigned caseload at least twice monthly. Practice good financial stewardship of agency and contract resources, including how time is spent, supplies are used, and property is maintained. Monitor spending of petty cash and other agency funds to ensure accountability and appropriate use. Meet identified key performance indicators and metrics for the position as identified for the program, division, and agency. Participate in discharge planning and write summaries of treatment as needed for client referral and releases of information. Provide immediate crisis intervention and resolution as needed. Always maintain client and staff confidentiality. Provide appropriate levels of treatment, skills training, and assistance to clients managing mental health symptoms and developing skills to manage symptoms. Partner with the team to deliver current programming and fill vacant position duties. Work collaboratively with the larger outpatient team of providers to support clients in actively working toward treatment goals. Coordinate care such as scheduling, attending, and arranging transportation for appointments and other case management needs. Maintain and document in client charts and program trackers following agency, county, state, and federal guidelines. Provide and receive input as part of the client’s care team on service plans and progress, and collaborate with other providers such as primary care providers and housing staff for housing transitions. Facilitate regular group and individual activities, encounters, and specialized client events, such as outings and holiday gatherings. Provide after‑hours crisis intervention with enrolled clients, when assigned: Answer all incoming calls and make contact within 2 hours of first call. Provide crisis response for clients referred by Crisis Connections. Complete assessments such as mental status, safety, danger to self and others, as appropriate for the situation. Coordinate with other professionals—including the care team, emergency room, social workers, police, family—to determine the least restrictive resolution. Provide all clinical information regarding the crisis to the treatment team by 9AM on the next working day and complete documentation per required timelines. Perform other duties as assigned. Qualifications Bachelor’s degree in social science or related field, or 2+ years of relevant experience serving adults with severe and persistent mental illness and/or in diversion services in lieu of a degree. For the FHARPS program, credentialing as a Certified Peer Counselor is acceptable and preferred. Apply for Agency Affiliated Counselor credential with Washington Department of Health within 30 days of hire, if not otherwise licensed, and obtain and maintain registration and/or licensure. Successfully pass criminal background check before hire date. Obtain/maintain a valid First Aid/CPR card or certificate within 30 days of hire. Hold a valid driver’s license for 1 year with a clean driving record to use agency vehicles. Have legible handwriting and ability to do simple mathematics. Able to navigate and comprehend charts in the electronic medical record. Able to work independently and within a team. Able to communicate information and ideas clearly so others understand; able to exchange accurate information in these situations.
