Seattle, WA, USA Job Description Asian Counseling & Referral Service (ACRS) has been working toward social justice and offering a broad array of behavioral health programs, human services and civic engagement activities for Asians, Pacific Islanders, and other underserved communities in King County and beyond. Position: Vietnamese speaking Case Manager I (full‑time) Department: Aging and Adult Services Key Responsibilities: Work with clients to develop and implement a person‑centered, holistic, and community‑based care plan. Conduct comprehensive in‑home functional assessments and re‑assessments, verifying diagnosis, skin condition, medications, treatments, psych/social information with health care providers as needed. Develop, implement, monitor, and modify a written, personalized care plan in collaboration with client/family, health care providers, and other community resources. Identify client needs and actively facilitate access and linkages to community resources, managing follow‑up referrals to ensure services meet client needs. Authorize services and client’s choice of qualified provider based on the established care plan; travel to and from home visits and make telephone contacts to ensure proper implementation. Authorize or re‑authorize payment for client‑approved Medicaid‑funded services and manage service changes or terminations. Advocate for clients in areas such as citizenship, family issues, funeral services, housing, food assistance, medical resources, financial assistance, legal advocacy, personal care services, translation/interpretation. Facilitate client transitions to various care settings, including Nursing Homes, Assisted Living facilities, and Adult Family Homes, coordinating with agencies to ensure appropriate placement and continuity of care. Act as a mandatory Adult Protective Services (APS) reporter and complete timely reporting of observed or reported suicidal ideation or other mental/behavioral issues. Assist clients and families with home modifications, provide short‑term crisis intervention, and transfer cases to appropriate residential care settings. Complete documentation within established timeframes, participate in quality assurance and process improvement initiatives, and meet program performance and productivity standards. Attend and participate in all agency, program, and team meetings, consultations, and in‑service trainings. Qualifications: Education and Experience: Master’s degree (social work, behavioral or health sciences preferred) with bilingual or bicultural skills. Bachelor’s degree (social work, behavioral or health sciences preferred) with bilingual or bicultural skills. Associate of Arts/Associate of Science with one (1) year of paid on‑the‑job social services, behavioral or health sciences preferred, and bilingual or bicultural skills. High School Diploma and three (3) years of paid on‑the‑job social service, behavioral or health sciences experience and bilingual or bicultural skills. Completion of Case Manager training program including State‑sponsored CORE Training is beneficial. Cultural Humility: Demonstrates respect and sensitivity when serving individuals from diverse cultural, racial, linguistic, and socioeconomic backgrounds. Delivers care responsive to each client’s cultural beliefs, values, and practices, contributing to a safe, inclusive, and welcoming environment. Knowledge and Skills: Effective communication with individuals of diverse backgrounds and perspectives. Proficient customer service skills with empathy, compassion, understanding, and patience. Strong time management and problem‑solving skills, adapting to constant change and thinking critically. Conflict resolution skills to mediate between clients and caregivers or family members. Knowledge of the aging process, aging network service delivery system, and other community service delivery systems. Familiarity with common adult illnesses and their impact on daily tasks. Ability to make sound mental judgments regarding safe environments during client assessments. Fluency in one or more Asian and Native Hawaiian/Pacific Islander languages preferred. Multilingual and multicultural communication skills highly valued. Technical Skills: Proficient in using modern workplace technology, collaboration tools, communication platforms, and business applications for document creation, scheduling, and data management. Additional Requirements: Reliable transportation and a valid driver’s license if using a vehicle for job duties; personal vehicle insurance required. Follow all agency policies and procedures as outlined in the Employee Handbook. Working Conditions: Regular mobility within the office or workspace; may involve sitting for extended periods; ability to handle objects or tools. Occasional lifting up to 20 pounds. Tasks benefit from visual acuity. Work Arrangement: Hybrid – approximately 60% on‑site (including home visits) and 40% remote. Hybrid expectations may change at the agency’s discretion.
