Moms In Motion is seeking a Service Facilitator (SF) Case Manager in Virginia. You will visit client homes locally, help families navigate Medicaid waiver programs (CCC+, CL & FIS, EPSDT), and develop plans of care to support safe, healthy, independent lives. No degree required; we value heart and experience.
