Moms In Motion is hiring a Service Facilitator Case Manager in Virginia. You’ll visit families at their homes, help them navigate Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and support safe, healthy, independent living. No degree is required; if you’re compassionate, organized, and community‑focused, you’ll fit right in.
