ILS, along with its affiliated health plans known as Florida Community Care and Florida Complete Care, is committed to promoting a higher quality of life and maximizing independence for all vulnerable populations./ppThe BH Clinical Care Specialist plays a crucial role in providing comprehensive behavioral health support to patients, ensuring they receive the highest quality of care. This position involves assessing patient needs, developing individualized care plans, and coordinating with multidisciplinary teams to facilitate effective treatment. The specialist will also monitor patient progress and make necessary adjustments to care plans based on ongoing evaluations. By fostering a supportive environment, the specialist helps patients navigate their mental health challenges and promotes overall well-being. Ultimately, the goal is to empower patients to achieve their health objectives and improve their quality of life./ppMinimum Qualifications:/pulliBachelors degree in psychology, social work, nursing, or a related field./liliValid state licensure or certification in a relevant behavioral health discipline./liliAt least two years of experience in a clinical or healthcare setting, preferably in behavioral health./liliRelevant experience may substitute for the educational requirement on a year-for-year basis./li/ulpPreferred Qualifications:/pulliMasters degree in a relevant field./liliExperience with electronic health record (EHR) systems./liliFamiliarity with evidence-based practices in behavioral health treatment./li/ulpResponsibilities:/pulliConduct thorough assessments of patients behavioral health needs and develop tailored care plans./liliCollaborate with healthcare professionals, including psychiatrists, psychologists, and social workers, to ensure comprehensive patient care./liliMonitor and document patient progress, adjusting care plans as necessary to meet evolving needs./liliProvide education and support to patients and their families regarding treatment options and coping strategies./liliParticipate in case management meetings and contribute to the development of best practices in behavioral health care./li/ul/div
