Job Description
Licensed Practical Nurse (LPN)

We are seeking a skilled and experienced Licensed Practical Nurse (LPN) to join our team. The ideal candidate must possess previous long-term care (LTC) and post-acute care (PCC) experience. This position requires a quick adaptation to the work environment, so candidates should be prepared to hit the ground running, as extensive orientation will not be provided.

Key Responsibilities

Provide high-quality nursing care to residents in a long-term care setting.
Administer medications and treatments as prescribed by the healthcare provider.
Monitor and document patient health status and report any changes.
Assist in daily living activities and promote optimal health and wellness for residents.
Collaborate with the healthcare team to ensure comprehensive care delivery.

Qualifications

Must have previous experience in long-term care and post-acute care settings.
Active and unrestricted Licensed Practical Nurse (LPN) license.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Flexibility to adapt to changing situations and priorities.
