Licensed Practical Nurse (LPN) - Homecare Travel Assignment in Fort Worth, TX 76107

We are seeking an experienced Licensed Practical Nurse (LPN) for a 13-week homecare travel assignment in Fort Worth, Texas. This on-site homecare opportunity is ideal for a compassionate clinician who enjoys delivering skilled nursing care in patients' homes while providing patient education, support, and high-quality clinical care. Located in the vibrant 76107 area, Fort Worth offers a welcoming community, convenient amenities, and a blend of city energy with neighborhood charm.

Schedule: Monday-Friday, 8:00 AM-4:30 PM
Shift Duration: 8 Hour Days
Shifts Per Week: 5
Hours Per Week: 40
Duration: 13-week assignment
Weekend Requirement: One weekend per month

Job Requirements

At least 1 year of home health field clinician experience required
Active Licensed Practical Nurse (LPN) license
Active driver's license
Current CPR certification
Ability to provide skilled nursing care in a homecare setting
Homecare Homebase (HCHB) experience preferred

Responsibilities

Provide skilled homecare visits to patients throughout the Fort Worth service area
Deliver patient education to support care plans, disease management, and health outcomes
Perform wound care, including wound vacs
Complete venipuncture and assist with PICC line-related care
Support care involving pleurX drains and other skilled nursing procedures
Document care accurately and communicate effectively with the interdisciplinary healthcare team
Maintain professionalism, compassion, and patient-centered care during each home visit
