Travel Hospice RN Case Manager Opportunity – Colorado

Are you a compassionate Hospice RN looking for a rewarding travel assignment? We are seeking an experienced Hospice Case Manager RN for a 13-week contract in beautiful Colorado!

Location: Montrose, CO
⏳ Contract Length: 13 Weeks
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Rotating on-call and weekend coverage

Requirements:
✅ Active RN License
✅ Minimum 3 Years of Nursing Experience
✅ Hospice Experience Preferred
✅ Strong Assessment and Critical Thinking Skills

Responsibilities:
• Manage an assigned hospice patient caseload
• Coordinate care with the interdisciplinary team
• Provide compassionate end-of-life care and symptom management
• Educate patients and caregivers on disease progression and care plans
• Conduct comprehensive patient assessments
• Deliver care in patients homes, assisted living facilities, nursing homes, and other residential settings
