Advanced Practice Nurse - Primary Care

Summary This is an open continuous announcement. Applications will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available. Incumbents may serve as a Nurse Practitioner in a Patient Aligned Care Team (PACT) setting. The incumbent will be responsible for providing medical care in an outpatient setting following PACT principles. The Education Debt Reduction Program (Student Loan Repayment) is approved. Duties Provides comprehensive primary care to a panel of assigned patients using the Patient Aligned Care Team (PACT) model of care delivery. Interfaces with non‑VA providers (Department of Defense and community) to ensure smooth transitions in care. Contributes to achieving performance‑measure clinical outcome targets in the areas of access to care, quality of care, efficiency, process improvement, and patient satisfaction. Prescribes medications and enters consults to specialty clinics utilizing electronic medical record systems. Practices in accordance with professional standards, core values, and a commitment to excellent service. Completes clinical reminders for chronic disease management and prevention when due. Writes progress notes, encounters, secure messages and views alerts according to VHA timeliness standards. Provides and coordinates clinical‑related employee health services in conjunction with the Human Resources department. Assists in completing Environmental Agents Registry exams. Demonstrates proficiency in practice planning and self‑direction in goal setting for managing each veteran’s medical needs pursuant to the scope of practice for primary care. Communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Provides customers with consistent information according to established policies and procedures. Handles conflict and problems constructively and appropriately. Protects printed and electronic files containing sensitive data in accordance with the Privacy Act of 1974 and other applicable laws, regulations, VA statutes, and VHA policy. Uses word‑processing software to store and retrieve electronic documents and files, activate printers, and edit text. Uses the Veterans Health Information and Technology Architecture (VistA) to access information in the Medical Center Computer System. Benefits and Qualifications Pay: Competitive salary with regular increases and potential for performance awards. Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid federal holidays). Retirement: Traditional federal pension (five‑year vesting) and federal 401(k) with up to 5% contributions by VA. Insurance: Federal health, vision, dental, term life, and long‑term care coverage (many federal insurance programs can be carried into retirement). Licensure: One full and unrestricted license from any U.S. state or territory. Work Schedule: Monday‑Friday 7:30 AM‑6:00 PM; variable tours and compressed/flexible schedules. Telework not authorized. Education Debt Reduction Program (EDRP): Eligible for a student‑loan payment reimbursement program. Must meet eligibility requirements per VHA policy and submit an application within four months of appointment. Award amount up to $200,000 and eligibility period one to five years.
