CareCore at Heritage is seeking a compassionate and dependable Nurse (RN or LPN) to join our Intermediate Care Facility (ICF) team on 3rd shift .

Our ICF setting provides specialized, ongoing care and support for individuals with intellectual and developmental disabilities. As an ICF Nurse, you’ll help support each resident’s medical needs, daily well-being, independence, and overall quality of life as part of a dedicated interdisciplinary team. This position is specifically within our ICF setting and is separate from nursing opportunities on our Long-Term Care side.

Schedule

3rd Shift: 10:00 PM – 6:00 AM

Responsibilities

• Provide nursing care and support to residents within our ICF setting

• Administer medications and treatments as ordered

• Monitor residents and promptly report changes in condition

• Complete accurate and timely nursing documentation

• Coordinate with physicians and members of the interdisciplinary team

• Respond appropriately to resident needs and medical concerns

• Maintain a safe, respectful, and resident-centered environment

• Follow all facility policies and applicable state and federal regulations

Requirements

• Current RN or LPN license in the State of Ohio

• Current CPR certification

• Strong communication and documentation skills

• Dependable and comfortable working independently during overnight hours

• ICF, developmental disabilities, long-term care, or related healthcare experience preferred
