Nurse Practitioner/Physician Assistant

Brooklyn, NY $140,000 - $160,000 Join a growing primary care practice based in Williamsburg providing telehealth visits for patients in assisted living facilities. This role offers a stable, supportive environment and the opportunity to build ongoing relationships with a recurring patient panel. (Not a remote position.) Responsibilities: Provide comprehensive primary care services to geriatric patients via telehealth. Conduct virtual visits with medical assistant support at each assisted living facility. Review and interpret lab results and diagnostic data collected onsite. Maintain accurate and timely documentation using established templates. Collaborate with other PAs, NPs, and medical staff to ensure continuity of care. Deliver high-quality, compassionate care to patients within an organized telehealth system. Qualifications: Licensed PA or NP in New York State. Credentialed with New York State Medicaid and Medicare (preferred).
