Activa Home Health is seeking a clinically strong PRN Physical Therapist (PT) to support our Medicare Skilled Patients and our Hospital at Home Program. Provide Medicare/Oasis and hospital-level PT care in patients homes focusing on mobility, balance, fall prevention, and discharge readiness.

Job Responsibilities

• Provide skilled physical therapy services to patients receiving hospital-level care at home and for Medicare Skilled Patients.
• Perform comprehensive PT evaluations addressing: strength, balance, endurance, and functional mobility; gait deviations and fall risk; transfers, bed mobility, and stair negotiation; activity tolerance and cardiopulmonary response.
• Develop and implement individualized, evidence-based treatment plans aligned with acute recovery and discharge readiness.
• Address functional decline related to recent hospitalization, acute illness or injury, deconditioning and mobility loss.
• Educate patients and caregivers on safe mobility techniques and transfer strategies, fall prevention and home safety, use of assistive devices and mobility equipment.
• Monitor patient progress closely, adjust plans of care, and communicate changes promptly with the care team.
• Collaborate with physicians, nursing, OT, and care management to support safe transitions and reduce hospital readmissions.
• Complete timely, accurate documentation in compliance with Medicare, hospital-at-home, and agency standards.

Qualifications Required

• Active Physical Therapist (PT) license in the state of Florida
• Degree from an accredited Physical Therapy program (DPT preferred)
• Current CPR certification
• Valid drivers license and reliable transportation
• Strong clinical reasoning, communication, and time-management skills

Preferred / Highly Desired

• Experience in one or more of the following: acute care or hospital-based physical therapy, hospital at home or transitional care programs, Medicare Skilled home health physical therapy
• Skilled in: gait training and balance rehabilitation, fall risk assessment and prevention, functional mobility and discharge planning, post-acute and medically complex patient care
• Comfortable practicing independently in a home-based acute care setting
• Familiarity with EMR documentation systems
