Bilingual Care Coordinator - Chronic Care Team

Suvida Healthcare is seeking a Care Coordinator to deliver high-quality care for Medicare-eligible Hispanic seniors. You will work closely with the Nurse Care Manager to manage daily follow-ups, DME/home health orders, triage calls, and hospital record retrieval, along with other clinical clerical tasks.
