Registered Nurse (RN) - Home Hospice and Palliative Case Manager -0.5 FTE- Days

Overview The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment. Assesses, plans, facilitates and evaluates timely coordination of high quality, cost‑effective care. Responsible for improving the health of patients dwelling in their own homes. This role requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills. The RN Case Manager provides hands on care in the home environment and offers autonomy, flexibility and the luxury of caring for one patient at a time. What You Will Do Collaborate with patients, social workers, physicians, other practitioners, caregivers and community agencies to coordinate and oversee the interdisciplinary plan of care for a patient in a home care/home hospice environment. Assess, plan, facilitate and evaluate timely coordination of high quality, cost‑effective care. Improve the health of patients dwelling in their own homes. Work independently, teach, think critically and communicate effectively. Provide hands‑on care in the home environment with autonomy and flexibility, caring for one patient at a time. Qualifications Associate’s Degree in Nursing required. Bachelor’s Degree in Nursing preferred. Current State of WI or eNLC compact state RN license. Valid State of WI driver’s license with an acceptable driving record. Current BLS (CPR) certification or ability to obtain upon hire. Clinical experience preferred.
