Advanced Practice Provider, Nurse Practitioner or Physician Assistant

NJ Palliative Care Solutions is seeking a compassionate and experienced Advanced Practice Provider, Nurse Practitioner or Physician Assistant, to join our growing palliative care team in a hospital-based setting.

NJ Palliative Care Solutions proudly partners with leading healthcare organizations, including RWJ Barnabas Health and Complete Care, bringing high-quality, patient-centered palliative care to patients and families across New Jersey.

Responsibilities

The Advanced Practice Provider will be responsible for delivering comprehensive palliative care to patients in a hospital environment. Responsibilities include:

• Conducting patient assessments and developing individualized care plans
• Managing ongoing treatment needs in collaboration with the interdisciplinary team
• Providing education and support to patients and families regarding goals of care
• Collaborating with physicians, hospital staff, facility teams, and community partners to support continuity of care
• Documenting clinical encounters in accordance with regulatory and organizational standards
• Promoting compassionate, patient-centered care for individuals with serious illness

Qualifications

• Active and unrestricted New Jersey Nurse Practitioner or Physician Assistant license required
• Active RN license required for APN candidates
• Valid DEA required
• Valid CDS required
• Current BLS and ACLS required
• Minimum of 2+ years of palliative care experience required
• Palliative care certification strongly desired, but not required
• Experience in geriatrics, internal medicine, hospice, oncology, critical care, or a related field preferred
• Strong communication, interpersonal, clinical judgment, and critical thinking skills
• Commitment to compassionate, patient- and family-centered care

Compensation & Growth Opportunities

NJ Palliative Care Solutions offers competitive compensation with bonus potential. Candidates with 10–15+ years of dedicated palliative care experience may qualify for compensation at the higher end of the pay range.

Highly experienced APPs may also be considered for Team Lead opportunities, offering enhanced responsibilities and additional compensation.
