Overview

Key Responsibilities

Perform comprehensive history & physical exams
Review medications and care plans
Complete STAR & HCC documentation
Document visits same day in EMR
Prescribe treatments within scope
Educate patients and families
Support care coordination and reduce avoidable admissions

Qualifications

Active Hawaii PA License
Minimum 1+ year Primary Care experience preferred
Strong communication and clinical skills
