Responsibilities
- Conduct comprehensive evaluations of residents presenting with speech, language, cognitive-communication, and swallowing disorders; develop individualized treatment plans.
- Provide evidence-based, one-on-one and group therapy sessions; document progress and adjust plans as needed.
- Perform screeners and assessments, create clear treatment goals, and track outcomes to support resident needs and care planning.
- Collaborate with an interdisciplinary team (nursing, OT/PT, social work) to optimize communication and swallowing care; participate in care conferences.
- Educate residents, families, and nursing staff on communication strategies and safe swallowing techniques; support discharge planning and transition of care.
- Maintain accurate, timely documentation in the electronic health record and ensure compliance with regulatory and facility policies.

Requirements
- Active Speech-Language Pathologist license in Pennsylvania or eligibility to practice in PA
- Master’s degree in Speech-Language Pathology from an accredited program
- ASHA Certificate of Clinical Competence (CCC-SLP) preferred
- 1 year of clinical experience in acute care or long-term care settings preferred
- Ability to work effectively in a long-term care setting and meet scheduling requirements
- Strong communication and documentation skills
