Provide evidence‑based, one‑on‑one and group therapy sessions; document progress and adjust plans as needed. Perform screeners and assessments, create clear treatment goals, and track outcomes to support resident needs and care planning. Collaborate with an interdisciplinary team (nursing, OT/PT, social work) to optimize communication and swallowing care; participate in care conferences. Educate residents, families, and nursing staff on communication strategies and safe swallowing techniques; support discharge planning and transition of care. Maintain accurate, timely documentation in the electronic health record and ensure compliance with regulatory and facility policies.
