Responsibilities: Perform PRN/focused and routine assessments of high risk members. Collaborate with primary care providers to develop and implement individualized care plans. Address, identify, and reassess appropriate levels of care, including non-medical support to ensure treatment compliance. Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals.
