Position: Nurse Practitioner (APN) – Internal Medicine
Location: Total Patient Care, LLC
Job Type: Full-Time 
Reports To: Supervising Physician
 
Position Summary
Total Patient Care, LLC is seeking a compassionate and experienced Nurse Practitioner (APN) to join our Internal Medicine practice. The ideal candidate will provide high-quality primary care to adult patients while working collaboratively with our physicians and clinical team to promote exceptional patient outcomes.
 
Responsibilities

• Evaluate, diagnose, and treat acute and chronic medical conditions.
• Perform annual wellness visits, routine physicals, and follow-up appointments.
• Order and interpret laboratory and diagnostic tests.
• Prescribe medications and develop individualized treatment plans.
• Educate patients on disease prevention, medications, and healthy lifestyle choices.
• Managing chronic conditions.
• Coordinate care with specialists and other healthcare providers.
• Maintain accurate documentation in the electronic medical record (EMR).
• Participate in quality initiatives and value-based care programs.

Qualifications

• Current New Jersey APN license.
• National Board Certification (AANP or ANCC).
• Current DEA and CDS registration.
• BLS Certification.
• Minimum of 1-2 years of experience in Internal Medicine, Primary Care, or Family Medicine preferred.
• New graduates will gain experience and mentorship from our established professionals on staff.
• Strong clinical judgment, communication, and patient education skills.
• Experienced Nurse Practitioners will benefit from our vast patient pool and knowledgeable staff of professionals serving the NJ community for over 30 years.
